As an Advanced Clinical Practitioner on the Wards in Sittingbourne Community Hospital you’ll join a team of support workers/HCAs, physiotherapists, occupational therapists and nurses who are focused on exceeding the needs of patients within our North Kent community. This role covers both services in Swale, you may need to work in both Sittingbourne Community Hospital and Sheppey Community Hospital.You will support the delivery of care to a range of complex patients which will include: life limiting conditions like diabetes, in dwelling long term catheter, urgent wound care etc.This is a weekday service, working Monday - Friday, 9am-5pm.
