* Location: North Wales
* Duration: 4-month contract, with the potential to extend
* Schedule: Full-time (part-time applicants welcome)

* Rate of Pay: �26-30 per hour, depending on experience

What You'll Be Doing:
As a Band 6 Occupational Therapist, you will be working within a dynamic and supportive Acute Frailty Team. Your responsibilities will include:
* Conducting comprehensive assessments for frail and elderly patients
* Developing and implementing personalized rehabilitation and care plans
* Working with a multidisciplinary team to enhance patient outcomes
* Delivering evidence-based interventions to promote independence and improve quality of life
* Ensuring timely and accurate documentation of patient progress and outcomes
* Providing expert advice and guidance to patients, families, and carers
Requirements & Qualifications:
To be successful in this role, you will need:
* A current and valid HCPC registration
* Previous experience working within a frailty or acute care setting
* Strong assessment and treatment planning skills
* Excellent communication skills and the ability to work collaboratively with a multidisciplinary team
* Experience in delivering rehabilitation and promoting independence for elderly patients
* A commitment to delivering high-quality patient care
Working Hours & Contract Duration:
This role offers a flexible working arrangement, with full-time hours required but part-time applicants are also encouraged to apply. The contract duration is 4 months, with the potential to extend based on performance and service needs.
