What You'll Be Doing:
Join a dynamic and supportive team providing community-based occupational therapy services. In this role, you will:
* Assess and manage a diverse caseload of patients within their home and community environments.
* Provide personalised therapy plans and interventions aimed at improving functional independence and quality of life.
* Work collaboratively with a multidisciplinary team to ensure a holistic approach to patient care.
* Engage with patients to promote rehabilitation, recovery, and support their ability to manage daily activities.
* Document patient progress and adapt therapy plans as needed to meet individual goals.
Requirements & Qualifications:
* Current HCPC registration as an Occupational Therapist.
* Experience in a community setting, ideally within adult services.
* A valid UK driving licence and access to a car is essential for this role.
* Strong communication skills and the ability to work autonomously and as part of a team.
* Passion for delivering high-quality patient care and helping individuals regain independence.
