Home first is the ideal outcome for many patients leaving the hospital and providing more care in a person’s own home is preferred, but a temporary stay in a setting that that can provide 24-hour care is sometimes required. The Recovery Hub does this, enabling early discharge from the hospital and reducing hospital admissions. The hub delivers complex rehabilitative care for the older adults in Leeds and the nursing care is delivered alongside a wider multi-disciplinary team. The focus always is on the individual, helping them progress to going home.We provide training, clinical supervision, and access to a peer support network. Wharfedale Recovery Hub service works on a shift pattern and covers 24 hours, 7 days a week.Community nursing in the recovery hub suits people who can work independently and within the wider multi-disciplinary team. You’ll be resilient, motivated, and adaptable to working in an ever-changing environment and of course, a passion for providing the best possible care to all. You will have great communication skills and maintain a high standard of clinical skills when carrying out nursing procedures.
