You will develop a personalised, wrap around support plan in partnership with the person you are working alongside, providing support in line with our organisational values and principles, assisting individuals to develop skills for independent living. You will work closely with our partners across the county and take a multi-agency approach to reach positive outcomes for individuals.Excellent interpersonal skills and an understanding of the issues facing those who are using our services are essential requirements for this role.
