The Community Matron Service is looking for a dedicated Case Manager to play a vital role in preventing unplanned hospital admissions and improving patient outcomes.

As part of our compassionate team, you’ll collaborate with Locality Community Matrons to assess, plan, and deliver holistic care. Your role will include clinical assessments, empowering patients to understand and manage their health, and providing support during exacerbations of long-term conditions until stability is restored.

You’ll work closely with a wide range of services—including community care, primary and secondary healthcare, NWAS, social care, and third-sector organisations—to ensure seamless care coordination. Together, we’ll enhance patients’ well-being through education, treatment, and access to essential resources.

Main Responsibilities
Assess, plan, implement, and evaluate care in partnership with patients, carers, and families.
Deliver evidence-based clinical care within patients’ homes, promoting self-management and independence.
Develop personalised care plans, including medicines management, to optimise health and well-being.
Monitor and evaluate care plans, providing timely interventions to prevent hospital admissions.
Work collaboratively with healthcare providers, social care, and third-sector organisations to deliver integrated care.
Attend and contribute to multidisciplinary team (MDT) meetings to support case management.
Provide education and health promotion to patients, carers, and colleagues.
Support students and junior staff through mentorship and training.
Participate in clinical audits and contribute to service development.
Adhere to safeguarding policies, professional standards, and governance requirements.

The Ideal CandidateWe’re looking for a passionate and skilled professional who meets the following criteria:

Essential:


Registered General Nurse with current and valid NMC registration.
Relevant professional or clinical qualifications.
Evidence of continuing professional development.
As this role requires travelling across our localities you are required to have a Driving License and access to a car for work purposes.
Experience providing care in patients’ homes or community settings.
Strong knowledge of long-term condition management and frailty.
Proven clinical reasoning, assessment, and treatment skills.
Effective problem-solving and decision-making abilities to manage risk.
Excellent interpersonal skills with a commitment to person-centred care.
Up-to-date knowledge of NHS issues, Clinical Governance, and their application in nursing.
Ability to work autonomously and as part of a team.
Strong time management, organisational skills, and resilience under pressure.
Committed, self-motivated, and adaptable.


Desirable:


Experience working with partners, third-sector organisations, and multidisciplinary teams.
Accredited learning in chronic disease management.
Effective teaching and presentation skills.
Mentorship qualification.


Additional Requirements:


Excellent IT skills.
Full driving license and access to a car.
Flexibility to work across all localities within West Lancashire.
