We're looking for a experienced Band 6 Community Physio to join our exciting Intermediate Care team here in Farnham Surrey. As a Physiotherapist in the team, you would be working closely with a team of skilled Therapist's, Nurses and Rehab Assistants to support patients in their own home to stay safe and independent. This may be following a medical crisis and assessing and trying avoid an admission into hospital or helping support with a long term condition and continues rehab.

Main ResponsibilitiesWithin this role, you will:
Support junior Physios, student and Tech's.
Deal with triage of patient referrals
Complete holistic initial assessments with patients
Liaise with family members/relatives and carers
Devise and reviewing care plans
Complete follow up visits with patients
Attend and participating in team meetings and in service training.
Be part of a team that responds to 2 hrs referrals as part of the UCR ( Urgent Care Response).
The Ideal CandidateThis is an opportunity to put your Degree in Physiotherapy to great use. You will have a full driving license with access to a car, you will be motivating, innovative and adaptable, with the ability to work flexibly across specialities as need arises, holding the HCPC (Physiotherapist) registration. With strong communication and organisation skills, and a flexible approach you will ensure delivery of positive experiences to patients at every stage of their healthcare journey.
You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, EMIS
