In this role you will play a vital part in delivering high-quality, patient-centered care to our diverse community. Working within a multidisciplinary team your experience in diagnosing, managing, and treating a wide range of acute and chronic conditions will be essential in ensuring positive health outcomes for our patients within the North Kent community. You will support the delivery of care to a range of complex patients which will include: life limiting conditions like diabetes, in dwelling long term catheter, urgent wound care etc.

This role covers both Dartford and Gravesend, you may need to work in both Livingstone Community Hospital and Gravesham Community Hospital.

Main ResponsibilitiesYour main responsibilties will include:


Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long term condition (as appropriate)
Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug based treatment methods into a management plan
Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice
Work with patients to support compliance with and adherence to prescribed treatments.
Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects, and interactions

The Ideal CandidateThe ideal candidate will have the following:


Recognised Nursing Degree Qualification (or equivalent)
Registered with the NMC
Masters in relevant area or working towards
Independent Nurse Prescriber
Advanced Clinical Assessment Skills
Experience with Frailty/older persons care
Evidence of achieving Autonomous Practitioner Competencies in Adult Nursing
Car driver and holder of clean UK driving Licence
