As a Band 6 Community Psychiatric Nurse (CPN) , you will be at the forefront of delivering exceptional mental health care. Key Responsibilities:

* Innovative Care Coordination: Develop and implement comprehensive care plans tailored to individual needs.
* Collaborative Teamwork: Work seamlessly with multidisciplinary teams to deliver integrated and effective mental health services.
* Risk Management: Conduct thorough risk assessments and ensure appropriate interventions are in place to safeguard well-being.
* Empowering Recovery: Support service users in achieving their recovery goals and maintaining their mental health.
* Exemplary Documentation: Maintain accurate and timely records in line with NHS standards and guidelines.

What We're Looking For:

* Experience Matters: Proven experience in community mental health settings is essential.
* Dependable & Proactive: A reliable and proactive approach to delivering high-quality care.
* Professional Qualifications: Registered Mental Health Nurse (RMN), Social Worker, or Occupational Therapist (OT) credentials.
* Excellent Communicator: Strong organizational, communication, and interpersonal skills.
* Current Registration: Must hold current registration with the NMC, HCPC, or relevant professional body.
