The Autism Outreach Service provides comprehensive support to children, young people, and families affected by Autism and learning disabilities across Essex.

We prioritise personalised and family-centred care, ensuring that individuals receive the right support at the right time, in a coordinated and community-based manner.

Our team of Autism Outreach Practitioners plays a vital role in delivering this vision, working closely with children, young people, and their families to empower them and facilitate access to essential services.

As a compassionate and dedicated Autism Outreach Practitioner the post holder will provide personalised support to children, young people, and families affected by Autism.

The Autism Outreach Practitioner will hold a caseload of individuals, working collaboratively with them and their families to develop personalised care plans, advocate for their needs, and ensure that they receive timely and coordinated support.

This role requires strong communication skills, creativity, and a commitment to empowering individuals to achieve greater independence and wellbeing.

Base

Essex-wide. 

Area based but agile to meet the needs of Children and Young People. Travel within the region may be required to fulfill the responsibilities of the role.

Main Responsibilities
Work with a caseload of children and young people to provide personalised support tailored to their needs and preferences. 
Engage with the child or young person and their family to develop a personalised, family-owned care plan that reflects their lived experience and preferences.
Ensure that care plans are flexible, responsive, and centered around the needs of the individual and their family.
Advocate on behalf of the child or young person and their family, facilitating open and transparent communication with other professionals and service providers.
That the voices of children, young people, and their families are heard and respected in all discussions and decision-making processes.
Collaborate with other professionals and agencies to access relevant resources and support networks.
Identify and develop community assets and signpost families to additional support services as needed.
Empower children, young people, and their families to apply solution-focused strategies and develop greater levels of independence. Encourage self-expression, autonomy, and resilience in all interactions.

The Ideal Candidate
Relevant undergraduate degree, or evidence of working at degree level.
Experience working with children, young people, and families affected by Autism and learning disabilities.
Strong communication and interpersonal skills, with the ability to engage effectively with individuals with diverse needs and backgrounds.
Empathy, compassion, and a commitment to empowering children, young people, and families.
Ability to work collaboratively as part of a multidisciplinary team and build positive relationships with stakeholders.
An understanding of safeguarding issues and child protection procedures, identifying risk and when safeguarding protocols needs to be followed
Driving licence and access to vehicle for work purposes


This is a full-time position based in Essex, with flexibility to meet the needs of Children and Young People.
