We have an exciting opportunity within our Community Neuro Rehabilitation team for a passionate and motivated Band 6 Physiotherapist, with previous Neuro experience, to join our emerging Swale service to provide specialist assessment, diagnosis, and therapeutic interventions for patients with diverse, complex, and complicated presentation and care needs, living in the community and to provide a specialist service to patients with neurological conditions across Swale.
In addition, you'll provide advice, support and education for patients and their families/carers, voluntary agencies and all those working with the patient regarding their medical condition and how this affects the person’s lifestyle and environment in order to maintain relationships, roles and independence.
This is a fixed-term part-time position working 21 hours per week until 30th March 2025. There is some flexibility on how the 21 hours can be worked across 3 days per week Monday - Friday.

Main ResponsibilitiesAs a Band 6 Neuro Physiotherapist you will:
Complete complex holistic assessments in the management of patients with chronic respiratory conditions.
Provide safe, clinically effective and timely intervention to maximize quality of life and reduce disabilities associated with neurologicalconditions.
Communicate effectively maintaining the highest standards of integrity with individuals (patients, relatives, other health care professional and the wider public) at all times.
Maintain accurate and legible patient records; in accordance with the organisation and HCPC standards.
Negotiate and plan with the person and carer, the goals and treatment plans whilst working with other professionals to achieve these. This will include leading and participating in meetings and case conferences in response to the needs of the individual.
The Ideal CandidateAs a Band 6 Physiotherapist you will:
Have a HCPC registration
Have a BSc/Diploma in Physiotherapy
Have previous Neuro experience
Have knowledge of relevant national guidance and other relevant initiatives
Have the ability to work in multi-disciplinary teams across health, social and voluntary sectors
Have excellent communication skills
Have motivation, adaptability and innovation skills
Need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
A full UK driving license and access to a vehicle for work purposes is essential for this position as it's Community based.
