As a Band 5 staff nurse you’ll be working as part of the Community Partnership with Frimley NHS system in Farnham. DATC deliver a variety of services to the local population of Northeast Hants and Surrey. Services provided include a one stop style assessment for older adults called a Comprehensive Geriatric assessment. Our aim is to improve patient outcomes, quality of life and support with anticipatory care needs.

The DATC consists of a collaborative multidisciplinary team who also work closely with partners to provide a holistic assessment of patient’s needs.

We offer a variety of diagnostic investigations including a Palpitations pathway which offers 24 hour and 7-day-a -week ECG monitors to patients aged 18 and over. We are also proud to deliver a therapy led Parkinson group, and will shortly be introducing a therapy led falls group.

Main ResponsibilitiesAs a staff nurse you will work closely with the wider Diagnostic and Treatment Centre team whilst also liaising with Administrators and Clinicians

You will:


Ensure that patients receive a friendly, efficient, and effective service.
Perform ECG recording, urinalysis , glucose monitoring
Ensure the highest standard of courtesy with all members of the public and multidisciplinary team
Maintain accurate patient records 
Have a positive attitude to change and a flexible approach to work.
Participate in team objectives and service development, team meetings and audits.
Follow all relevant health and safety policies and procedures 
Comply with mandatory and statutory training.
Be prepared to take the lead in new developments in the department

The Ideal CandidateYou will have:


A nursing degree and hold a valid NMC registration.
Minimum 3-year post reg experience
Previous community experience would be desirable, but not essential. 
Experience caring for older adults would be beneficial.
Strong ability to communicate by written, oral and electronic.
Good presentation and a friendly manner
An ability to motivate others and promote a professional customer care approach.
